Your records are confidential. If you wish to have copies of your medical records released to someone outside of our practice, please send us a written request authorizing us to release this information. You may also come into our office to complete a records release form. We require 48 hours notice to copy and mail, or fax, your records. Please keep our office informed of any change in your name, address, employer, telephone numbers, and/or health insurance coverage.
